Hi there, thank you for contacting SDG. You can exchange the heads always they are of the same brand and material. Shanaya's body has a moaning option and the other heads can have it too. Moving forward, the difference between the two options is Enhanced Mouth comes with teeth and tongue pre-molded fixed into the mouth of the doll. And, the Teeth and Tongue Set is removable and not permanently set.
